Cold approaching in bars/clubs?

The next couple can work off cold approach, but tend to work better for me off of warm approaches (I've forced or gotten an ioi from her already), than cold ones.

"I love that dress, nice heels too." (an obvious undertone of 'I like your body'. Shows more sexual confidence than other men there.)
"Hi, I'm Chad" (If she's thrown you an ioi, this is really enough. She checked you out, thats basically asking 'oo who is that?", so walk up and tell her who you are)
"These guys are just hovering around jesus, you're popular I guess. See any guys you like?" (don't be serious in tone. Shows social awareness. Separates you out from other guys)

Buy a drink and use that to say cheers to the group, and then you in. And then have a canned opener, that get most of the group to talk. "How lies more", hand reading...

Alternate, say cheers to every group in the club, and your social status will rise and girls will start to pick you up.

Do it two times in one hour, and you start to have rock-star value...

Compliment the outfit (if legitimately good) of an undesirable girl in the group. She'll be delighted and you'll basically be welcome into the group and be more important than any conversation that was going on prior because you're providing something so valuable to her. This allows you to get your foot in the door and just talk about other bullshit you'd normally talk about.
