If you can't tell, just flirt and escalate. How she responds to your advances (escalation during the conversation) will provide you with guidance on how to proceed. Keep practicing with multiple women and eventually you will sharpen your social skills so you will know if a girl is flirting, giving IOIs, or just being nice.

Also, you're going to have to get over the paranoia and just be BOLD and go for it sometimes. You will get rejected, it's a a part of life and not a big deal. The paranoia stems from a fear of rejection. Take risks and ask women out when you are talking to them. This will teach you that getting rejected isn't a bad feeling.
