Nice..all i can say is good luck. You and i almost share the same personality. As a matter of fact just last week i finally had the balls to ask my ex co-worker on lunch date in which she kindly accepted. So tuesday of this week, took her out and we had a good time. Later after i dropped her off, i asked her if i can take her out again and she said yes. Now here is the kicker i asked to let me know when she will be available and i still have no response from her....

Just suggest a time "Want to go___? I'm free _____". If she can't make it she'll suggest another time (if she's interested).

Don’t be yourself, be the best part of yourself. Be calm. Be confident. Give her what she liked before: that was you, you just weren’t nervous. You can do it. Close your eyes, slow your breathe and know you are that person already.
Assume it’s going to go bad. Assume you will have to try again with someone else. Failure is life, it is learning, it is experience. If you’re going to fail anyway, why worry about it? Go there and be the best you you can be; go to learn from it. I believe in you.

The first...congratulations...step by step you get far, even though you had liquid courage you interacted well and she made her move...ready on your part...the plan seems good to me, you need to encourage conversation...know more about her...imagine the appointment, set the stage in your mind...reproduce it....forget everything! you don't have to be a robot...you have to generate empathy in her but don't deceive her with someone you're not...imagine the situation of dominion of yourself but you can't pretend to dominate all the interaction because it would be something violent for her...with that aplomb you can already be sure of what you want from the encounter.

this will be the first date I've ever been on and I feel dread. I understand I have to just be myself, don't expect anything from it and remember she's a human just like me
Great! Nervousness is your humanity, show it off! You're just nervous about showing your nervousness! Don't place too much pressure on her being the one to complete your life, but it's really sweet when two people are so nervous for each other they can barely speak. The fact that she took the initiative in messaging you (women will very rarely respond when I leave my info; it's a lot to ask) means she's into you. All you have to do is show up, show interest in her, and gradually show who you are.

Don't try to please her, or be hounded by thoughts of "living up to her unknown standards"
Turn it around. It's you who's getting to know her to know whether you like her or not.
