You're both 15, you're both probably asking the same questions. When I was 15, I didn't know what I was doing, we're all learning when we're 15 mate, and in fact I still have a lot to learn. We learn on the job mostly.

With the little information I have, I can assume she also likes you back, so she's probably as nervous as you. As the man, though, I think it should be up to you to push things along so it's good you asked for advice. From what you tell me, there is clearly sexual tension, so perhaps you break the tension while talking, make her laugh a little, and then start getting a little closer to her once she's comfortable (humour puts all people at ease, not just girls) perhaps make small moves like arm round shoulder, maybe some hand holding. If she moves away before all this, then that's just a clear signal she's not interested and at that point you may as well think about ending it, but I doubt that'll happen. The increase in physical interaction could be over the course of a week, a day or even one interaction.

As for asking about making it official, it never really needs to happen, but strike while the irons hot, i.e at the end of a successful date, or at the end of a day when you two have gotten along particuarly well.

You first have to kiss her before you think about relationship. There is nothing you can make official if nothing has happened like a kiss is the bare minimum.

You need to go alone with her, flirt and go for a kiss, not ask her for a relationship.

First comes a kiss, and then tearionships. No relationship first and then kiss.

Go to a park after a date, grab her hand and walk around. Sit down on a bench where there's a little bit of privacy. Talk. Look at her eyes and stop talking. She'll get nervous probably. Kiss her. Don't use tongue immediately. If everything goes well, start frenching.
