What does this even mean to you?

Important decisions like change of daycare, school need to be shared decisions

And what's the tie breaker? Because at some point, you aren't going to agree. You can have language about binding mediation, but then you're just putting the decision in a stranger's hand.

Access to passport and medical records

You don't need the other parent's permission to access medical records. It's your responsibility to get access to the physician online portal.

Passports require both parents to sign in order for children to receive them.

Thursday evening-Friday morning, Sat/Sunday(alternate) morning 10 am to evening 8 PM. Summer 15 days(may), winter 15 days(December)

Artisinal schedule much? There are standard parenting time schedules: 7/7, 2/2/5/5, 2/3/3 -- all of which are dependent on the ages of the children. If your kids are at least 7yo, then 7 days with one parent then 7 days with the other parent. Holidays alternate each year. For example, the mother gets the first half of winter break during odd years (aka Christmas) and the father gets the second half of winter break (aka New Year). It flips in even years. You alternate each holiday like that so parents don't have back to back holidays. For example: Thanksgiving with dad, Christmas with mom, New Year with dad, Spring Break with Mom, Memorial Day with Dad, Labor Day with mom, Fall Break with dad. Rinse and repeat.

In the event of moving, the moving party must give up custody. The moving party gets 30 days in summer, 15 days in winter.

Delusional. The court will always maintain the right to determine custody. You can't bypass the court.

If one parent moves far enough away that it's a long-distance parenting plan, then that's what will get implemented. Every state has different thresholds for long-distance parenting plans. Some it's a certain amount of miles away. Realistically, if the parent can't drive the kids to/from school then it's gonna be a long-distance parenting plan. For the parent that moved away, essentially that's every other holiday and about 6 weeks in the summer. The parent that moves away is responsible for the travel costs of the children to/from them.

What you can do is create a geographic restriction which defines the school district in which the children will attend school. That aligns with the "best interest of the child". As long as one parent remains in that district, then the children will attend that district. So if she moves halfway across the country, the kids aren't moving with her.
