JavaMummy is right - one step after another. Trying to take too many steps at once is what makes people come off as a creep.

While you're making out, touch her skin. I mean, her neck, the back of her neck, that sort of thing. Then she's used to you touching her skin.

Then, lift up her shirt or whatever so her skin is exposed. At the same time, lift up yours so your bellies touch. Obviously you have to be subtle about it.

This is probably going to spark strong sexual feelings in her and should hopefully remove any last minute resistance - pretty much the only time she ever feels a guy's belly against hers is during sex. Read DiCarlo's Escalation Ladder for more.

She just needs to be coaxed into it. If you are already making out then the only missing piece is to get her turned on enough to want to escalate, and to have a proper venue for sex. I personally think that if you are making out with a girl correctly, the only steps between that and sex are time and a bed.

Grab her ass, kiss her neck, guide her body. Women are reactionary and if you take charge she will just be that much more turned on when you actually round 3rd base/get hard/penetrate/etc.

Be fearless, and make her want it before you move forward. Consent is given by body language and the purpose of foreplay is to elicit that body language.

Also, girls don't make out with guys they find creepy. Get that shit out of your head.
