I don't fully understand your question, perhaps it's because I haven't read DiCarlo's writing, but I feel I know what you're trying to ask. Perhaps this is an adequate answer.

Holding hands is not what you're making it out to be. It's not like you two are going to be walking around like a couple kids in junior high with your fingers interlocked.

Holding hands in conversation means loosely touching her hand in a way where you two are only making physical contact with your fingertips. It's seductive and flirtatious and will raise her heart rate. This does not mean you make an obvious lunge for her hand, rather, you light bump or grab hold of it nonchalantly. It's an easy step that makes sense once it happens.

If you're guiding her around, hold onto her hand as though you are a gentleman helping her down from a carriage, again fingers not interlocked, but grabbing hold of her fingers delicately.

Hand grabbing is one of the first things I do once I established some basic contact. While talking, or after a trigger like something funny she said or whatever, grab her hand, hold it for a couple seconds and let go. It's easy and in my experience gets your intentions across and builds attraction very quickly. Also, I fucking love holding/touching hands.

Going from touching, and having your hand on her leg to holding her hand is a much smaller gap, and very comfortable. Also if you are walking next to each other and have been kinoing before then it's pretty easy to just grab her hand when your arms start going in stride.

I will generally go for a hi-five, then interlock fingers and continue this while the hands are lowered and a few seconds after the hands have gone all the way down. So it is technically hand holding, but it isn't like walking down the street holding hands.
