Hi! I really enjoyed our last date! It's time we had another. I'll see you at the aquarium at 12 on Saturday. Call me back if you need to reschedule, otherwise I'll see you there.

If you say it as pleasantly as possible, this will work. The important thing to do is not give her an opportunity to reject at this point.
