I was just thinking about this the other day. This used to be a huge issue for me. A couple tips that might help:

When I'm in this spot we're usually are sitting next to each other on the couch, so I'll put my arm around her and play with her hair. After a couple minutes of this (not too long or you'll psyche yourself out) it's easy to just pull her in.
One issue I had was that I'm too much of a nice guy deep down. I wasn't so much scared of rejection, I just didn't want to feel like a jerk if they rejected me. If this is your problem, internalize the fact that not only is it not a big deal for her to reject you, but you might also want to view her as a sex object with no emotion. It sounds bad, but it can help.
I'm sure you've heard the 5 second rule, or whatever it is, for approaching. It's the same idea here. If you wait too long you won't be able to do it.
I got stuck in the conversation loop too. In my opinion it's best to do it right as she's finishing a sentence. Sort of like how racecar drivers rev their engines right before the light turns green, you can start the pull near the end of the sentence than as soon as she's finish finish it quickly (try not to do it so quick you headbutt her though.
Speaking of viewing her as a sex object, you might want to practice on girls who aren't super attractive and who you don't know very well. Back when I struggled, I was trying this with a girl I had known for years and was incredibly beautiful. Not a good combination. Where I first really succeeded was with a girl I had spent a grand total of 2 hours face to face with, and who I wasn't really that excited about. So if she rejected me and ran off it wasn't a big deal.

Overall, it's both the easiest and hardest thing in the world. Once you do it you'll shake your head and wonder why you got so worked up over it.

When I'm stuck in this situation I usually just say fuck it, and make a move as soon as possible. I know that sounds bland but really, if a girl wants to take you away from the crowd and have some one-on-one time she wants you to make a move just as much as you do. Doesn't matter when, or how for that matter. Just make sure you aren't a mong when you do it.
