Just don't marry the celery merchant and then neglect to visit back home for a long while. That npc is the one way to lose weight and after she runs off with chad, your character will blow up like Fat Bastard and there is literally nothing in the world that can stop it.
Watched it happen to an old roommate's playthrough. I laughed my butt off
