I would end it. You're having doubts, it's been 6 months and his behavior is putting you on edge. Why continue to make yourself unhappy being with him?

It feels better to be single than to deal with someone's uncertainty/lukewarm behavior.

Don't waste your time writing paragraphs about your feelings. He knows it's bad to not acknowledge the birthday of a woman he's been dating.
