Not so sure I see an issue with this.

You never know what you’re going to encounter in a patient, and if this style of ‘marriage’ is on the increase, then as a practitioner I’d want to be aware of the permutations and terminology so I can comfortably discuss it with my patient, plus any known or common issues that one sees with this style of relationship.

It’s not for me, in any capacity, but I’d be stupid to turn down money if someone in one of these relationships is willing to pay me, right?
