I've seen this kind of thing going on since the 1990s.

I don't necessarily have any issues if the couple were married as long as the terms of his will are not affected, the wife will probably get everything already. But if affects the terms of his will, it should not be done.

But if it's a bf/gf relationship I absolutely object, it's wrong under all circumstances..

And if the man is in a coma and might recover, I also absolutely object. Estate issues aside, I'm pretty sure that the sperm extraction process will leave him impotent and probably infertile if he does recover. I have seen perhaps 2 issues like this over the years.
