Original Case Incident Report states:
1) He had reported 3 previous abuse cases, further details reveals he had been previously stabbed and reported it on previous occasions
2) That he was "advised" of OOP and warrant procedures and a hotline #, which is not true as shown by video evidence and the fact that the cop shot him before saying anything
3) Confirmed that he was the one stabbed/cut

Tactical Response Report:
1) "Did not follow verbal instructions" Lie - The cop first shot once with no full visual or idea of what was going on, yelled "stop" AFTER shooting, and then shot a second time.
2) "Imminent threat of battery with weapon" Lie - It's proven and confirmed that he in fact was the one stabbed, and that the female had zero cuts on her, meaning she is the one that stabbed him.
3) "Physical attack with weapon" Lie - Once again, he did not attack her with the knife since she had zero cuts and he had cuts.
4) "Used force likely to cause death or bodily harm" Lie - The lethal force was towards him from the female, not the other way around as shown by video evidence and the autopsy.

"Viewed before completing report: None" - They completed the report without even caring to look at any video or anything, lul.
"Member actions recommended: None"
"Supervisor actions recommended: None" - No debriefing, no video review, no policy review, no training review, nothing done as a result of shooting and killing the victim who called for help, the victim who had 3 logged cases of reporting abuse against him and being previously stabbed.

Well. The cops fixed the domestic violence issue there. The seriously wrong way. But when you don't train people enough and hire moronic power mad twits. Then crap like this is going to happen.
