You should have been building to the kiss. Casual touching all evening, flirting and body language. The idea isn’t just to kiss, but make her want to reach that conclusion naturally. Sure, it’s going to be awkward if you waited all evening to make it a real date.
It costs you nothing to try for another date and if she consents then be a little more aggressive. She obviously wants you too.

I think that before you learn anything, you should focus on self awareness. Her leaving early, and rejecting your kiss send the clear message she is only looking at you as a friend. No amount of waiting will change that. Things will get better just practice.

Definitely contact her again. I feel like you are over thinking that moment she obviously wasn't completely put off as she wanted you to kiss her forehead.
Also situations are only awkward if you let them feel awkward. She will either say yes to seeing you again or no and then you move on to the next one.
From what you've said though it sounds like she is interested and would say yes to seeing you again.

If the kiss was the only factor then it's fine. Girls don't mind a bad kiss attempt as much as making no kiss attempt.
Looking at the entire picture based on your descriptions though it's best to leave this and move on. There seems to be too little sexual tension.
