Dude, you're saving cigarettes and if she doesn't want to flirt then she's not going to flirt anyway.

I don't smoke but if I did I'd certainly use that line.

I always use lines like that.

Usually what I do though is I start acting like she's a total drag, so I say "Ugh... I guess you can have a cigarette" (and I'll take it further when she needs a light or whatever) or if I'm in a situation where I know she can easily get one from someone else, I'll just say no but then start a conversation from there.

Your line's definitely good though. I'd guess the times it hasn't worked were pretty specific situations, and most likely when you didn't have a great frame. That sort of line should be good most of the time though if it's done right.

You said the second they're done smoking, they leave right? That means it only depends on how much the person wants a smoke. If they thought you were cocky, liked it and wanted more, they would stay (so it could work). I guess that could lead on though. If they're having a smoke with you, you could get into a conversation and try to impress her before she's done. If it works, you're going to get some more action. But to be honest, this seems more like a "you'll get a little bit of flirting, but don't expect much".

Now, if they're really desperate for a cigarette or think you're cute, they'll probably give you a kiss. That could start off potentially on a positive note, but as I said before, you probably won't get anything more than that unless you can impress them.

The good thing about this is, that you don't have much to lose. Someone said "Mmmm, not worth it". This means you keep the cigarette and the person leaves. You move on and wait for the next person. The only bad thing is that you sound a little creepy if it doesn't work out. But that's pretty much it.

Don't just rely on cigarettes though. That can get you only so far, but you really have to be confident and get a good conversation going. They're most likely just wanting a quick smoke and to leave, so I would rather go out if you wanted more. This is only my opinion though. I don't smoke and don't ever plan to, so this is from a guy's perspective that doesn't even smoke.

I'm a chain smoker so next time this happens, I'll try this line out. Seems good.
