I know you shouldn't break eye contact immediately like in the gif, but should you really hold until she breaks? Seems creepy. Wouldn't it be better to simply hold for a few additional seconds to show that you aren't insecure?

Fuck it. I hold eye contact until they look away uncomfortably. You can tell by how a chick looks away if she's attracted or not. If she's straight uncomfortable, not great, if she looks away bored or annoyed looking, not great. A plain lookaway is ok, but not awesome.
Best is when she looks away like you caught her checking you out.

My steps are usually
4a. (If in close proximity) Proceed to talking
4b. (If too far away or inconvenient to get over there) Make a face

"Creepy" is an ridiculous notion. At no point in time should you ever meter your actions in an attempt to not seem "creepy". Do your thing, with conviction and poise and don't give a fuck if someone might call you creepy.

I hold eye contact for a few seconds and smile. Judging by the reaction I'll move in to approach or move on to something else.

If she looks back, I usually hold eye contact for 3-5 seconds as a way of saying "i'm totally into you"; then I smile as a way of saying "and yup, you're totally into me" and look away (not up or down, but to the side) as a way of saying "but I have more important things to do". They're usually hooked after that :)
