Also I'm personal a big fan of "rejection therapy" - going around and just getting rejected. I have a lot of approach anxiety. The first couple times I went out for "day game" I did absolutely nothing because I was filled with fear.
Unfortunately the way to get rid of fear is to expose yourself to that fear. IMO the best crutch is to get friends or like-minded allies (you know, like wingmen) to encourage you to approach and exert social pressure. Unfortunately it's not an easy task to find people that can give you the proper encouragement.
Another strategy I personally use is to "take control" of the situation by explicitly going for rejections. Make your goal rejection. The nice thing about rejection is that it's a very attainable goal. Go to a cute girl and explicitly ask her out on a date. You're going to get a lot of no's. Mission accomplished. The goal of this exercise is to make sure you're actually approaching women, disregarding the ultimate outcome.
Work on reducing your approach anxiety by facing rejection head on and accepting it, embracing it. That's what's worked for me. Everybody's different so lots of people have different strategies for coping with approach anxiety.

I struggle with finding the perfect "time" to approach. And the perfect woman etc.

Some rule of thumbs you'll see around here.
Guys who only approach perfect 10's are also called virgins. Would you have sex with her? Yes? Then you need to approach her.
The perfect time to approach is right now. The very moment you see a woman you'd like to fuck, you need to go talk to her. And if you've missed that moment, the second best time to approach is now. The longer you dawdle, the harder it is, especially for newbies. See an attractive woman? Go right for her, as soon as possible. Real men know what they want and they go after it.
Your first step isn't about "pickup-artist mental strategy & tactics". Nope. Your first step is conquering your emotionally driven instincts, mainly your fear. It's OK to be scared. Most guys are.

1.Approach in a way so the girl don't get offended or creeped out by you.
2 If she somehow feels offended, the last thing you want her is to complain against you to the mall guard for disturbing/harassing her, and they will issue you a trespass notice for 3 months or more depending on the context of the complaint.
3.Keep the conversation simple as you can. If she rejects you, say, "nice talking to you, have a good day" and leave

I am not a fan because of the age difference I'm 36 most girls who go to malls are 18 and under so it's very rare to find a girl in my age range.... I do talk to moms and what, not just friendly chat, If I wanted to be sexual I would ask where the husband is or make a joke about it... "aww needed to let the husband play video games huh". A lot of PUA get kicked out of malls so buy a Jamba juice or carry around a bag.. good opener as well... hey I just saw you, while i was returning items.... blah blah blah
