Seems like both your faults. He should be asking when you’re free so you can both find a date for the date/meet-up. But, you’re also saying “you can’t” etc without giving a day you can, so when you are finally free his attractions probably lower/toned down a bit. If he is serious and doesn’t feel like you’re serious logically it doesn’t make much sense for him to go on a date.

Sounds like there's miscommunication from both ends here. If you keep saying you're busy without offering an alternative date, of course he's going to stop talking to you: he probably thinks you're not interested in him.
Maybe when you agree to these dates it wouldn't hurt to reach out to confirm with him that you're both still on if you haven't heard from him. Us guys are always taught to reach out and confirm plans the day of a planned date if the girl hasn't already. Remember don't expect anything from everyone, if you want confirmation you haven't got it's on you to get it yourself.

They keep talking to me but mention nothing about the plans. And then the cycle continues..

Can be so many things :
He's trying to get your attention and look like a busy guy. Or wants you to think he doesn't care as mich as you do.
You were his second option and he found something else to do.
He changed his mind about seeing you, could be because of something you did, or not maybe he's not in the mood anymore
Or he keeps you for validation
Anyway very few of the reasons are good sign, if he liles you he'll make time for you and respect you. Don't let them do that to you, it's impolite, it's rude and it's not respect

He definitely would. Not your job to confirm plans. Either make new plans and bail on him if you're not that interested, or keep the night open if you are.

I do it, actually just posted about it in my last comment on a diff post. When I do it, it's nothing personal. More of a: I lost the interest in hanging out/I just want to be alone. It could be a million reasons why really, but for me, it's usually the anxiety, preparation, and effort I have to put into hanging out with new people.
