the classic "Whats your name?". she answers kinda hurt then you say "nonono your last name", which is more socially acceptable.

I once # closed a girl at a bar by giving her my number. She texted me a week later. I was like, "Who is this?" Instead of giving me her name, she referenced a conversation we had. I was like, "Oh!" Then I asked her out. About three hours into the date we met some new people and she had to introduce herself. That's when I learned her name. lol.

You could also try, "hey, how do you spell your name so I can put it into my phone?" Haven't tried it myself, but I heard it works.

aight just came up with this -- early on when you are hanging out with her play this lil game: have her come up with an adjective for each letter of her name that describes her. Or better yet you come up with the adjectives and you can ask her to spell her name just to double check it. this way you can come up with silly fun stuff.
