How do you manage a "rotation"?

I'm dating 5 women right now (down from 6).

Generally, each girl will get 7pm until we go to bed, probably sleeping over, unless weekend in which case I like to get brunch or have them cook me breakfast ;)

I see each girl about every 7-10 days. Some girls I'll see 2-3 times a week depending on how schedules go, other weeks it might be 2-3 times with different girl.

Yes, they understand there are other girls and in many cases know them by name and how my dates / relationships are going. Full transparency.

If they want something serious, I explain how I feel about them, where I see the relationship heading, and if needed give a timeline for how changes in our relationship might go down.

Edit: Google Calendar is your friend. Write down EVERYTHING

I did this several years ago and while I found it to be exhausting, it does have one huge benefit. If you are trying to figure out what kind of girl you are most compatible with, this is the most time efficient way of doing it. Most guys waste years on only a handful of girls and end up in mediocre relationships or alone, whilst never truly meeting a girl that is right for them because they were in monogamous relationships. What I would do is tell each girl I only have about 10 minutes to talk and maintain their interest via the phone or IM and set up a few dates every week. Never more than 1 date with 1 girl per week though. The most I ever rotated at once was 5, but I found it quite annoying. Rotating 3 at a time is very easy and manageable. If you don't have a lot of free time, I would echo puaCurveBall's advice of using Google Calender, and if you have an iPhone, flip it horizontally to see your Google Calendar schedule visually on your phone. Seeing the timeblocks visually really helps. That's what it's all about: timeblocking.

I don't have much outer game skill, so this advice won't be like puaCB's, but I have worked for a long time cultivating my inner game. I think inner game is the key to dating multiple women, because you're exponentially multiplying your need to be congruent, outcome-independent, and in control.

I find that three at a time is all I can handle, but then I prefer to have closer relationships with women. The three would be meaningful relationships that go beyond just sex, and I'm often emotionally connected with the women I'm seeing.

My advice is try to see each at least once every two weeks, if not two or three times. Any less and the connection will go cold, or they will feel that you're being inconsistently interested in them. Communicate regularly, even if you're not seeing them all the time (I'm terrible at this, but working on it).

Time management becomes important, definitely mark everything out in your Google Calendar (I use it for all my non-dating social obligations as well). In order to have some semblance of a life on the weekends while I'm actively dating new girls (as opposed to maintaining), I set the ground rule that "weekends are for friends and family". So, I only see women that I've developed close relationships with on the weekend. Everybody else I see during the week.

1) Interaction is usually a typical date - 7PM through the next morning. I almost never stay at their place. Dates are essentially what I want to do, and I invite them along. Might be dinner, might be a concert, movie, etc

2) At least once per two weeks, preferably two to three times

3) I'm always 100% transparent. Most won't want to know the details on who else I'm dating, but I'm always up front that I'm dating other people. I do get into "serious" emotional relationships with multiple women, but if a girl asks for monogamy I generally just explain that it's not right for me right now. If this is at the start of dating, it's not a problem, we're just not right for each other. Sometimes it will happen later, in which case I have to decide whether she's worth changing my lifestyle for. This happened once.

puacurveball runs a tight ship. But I don't know if that ships for everyone as it's really hard to maintain and I was never one to really get into google calendars and such, I am much too spontaneous. I think what it means though, is you've built up strong rapport with multiple girls and because their caught up in your web, you can enjoy them whenever you like.

I have a 'bullpen', which I suppose is different from a rotation in that I just play whichever has a hot hand is interesting and available for the times I want to have fun. Interactions can last 20 minutes to days if I want to go on a trip or something with them.
At least once a month, it really just depends on her, me and what feels right. There are pros and cons for not seeing some of your relievers all the time, and truthfully, and this is a bit twisted, I see all 3 as guinea pigs, in which I test theories and strategies out on. I still try and meet new women, which is an important part of a bullpen/rotation, as you're going to need to maintain your roster, as there are always going to be reitrees, and just bad girls for you, so you need to always be restocking, or at least preparing to have someone that can fill the role.
I think they might assume it, but I never talk about it. I always just say I am busy with work or life which is true. It's better that way, yeah it's nice to get them when they are a bit jealous, but that jealousy can lead you down a very joyless path.

It depends on my situation, and my attraction to the girl. If the rotation isn't deep, I might feign developing more interest just to keep my dick in her, but there have been times when I just let them walk because I don't want to have that kind of a relationship and I am fine with it because I have other warm bodies in the bullpen who can get the job done.

I'm pretty jaded and it's the reason I want a rotation--and as amazing as it sounds to go from one girl to the next, it's a bit of a grind. It's fun, but it's not everything you hope it would be. I'd recommend giving it a go if you're interested in the idea but it's not for the romantics (like I used to be) or betas. It's just about the fucking.
