Girls, especially really attractive girls develop habits like this as a defense mechanism... Just think about how often a girl gets hit on during a day. A lot of times its subconscious, or a nervous habit.
Pay close attention to body language, are they in a full conversation on the phone? do they look like they're in a hurry? or are they just kind of fiddling around? Once you establish what it is they are actually doing knowing how to approach comes easier.
One example is if a woman looks super busy, shes bouncing from one spot to another grabbing things, or typing furiously, with a coffee in her had and a phone in the other. Just sit back and watch her... she will feel you looking at her, and most likely look up at you. Then you just say "not enough hours in the day huh?" If she has headphones in she'll pull them out, and ask what you said. If she doesn't under stand the reference she'll say something like "excuse me?" then you just follow with, "you just look so busy"

If she's in a public place (i.e. not the library for example... or a hospital berdoom!) it's not really reasonable to expect no interruptions, IMO. So it's okay to approach. If she truly wants to be left alone she'll let you know. Your focus at that point is just to find that out.

And just something I sometimes do: when I see a girl reading on the bus, and I get to sit next to her, I don't talk to her until she flips the page. Then say something like "hey I'm not finished with that page yet" and usually get a smile and go from there.

If she looks uncomfortable and busy, she has probably had enough attention that day and can't wait to get to where she can be comfortable. If you're a friendly person and can offer genuine interest and no pressure, by all means, do what you do! but otherwise, give it a pass and approach the next time. We build our walls higher in defense of those who don't get these cues. Don't make it harder for each other.

If you want to bad enough or if you think you've gotten an IOI go for it. But if you've tried establishing eye contact and they avoided your eyes or you in any way and they have headphones in or are on the phone you should know they're probably busy. Learn to read people's body language, you'll be able to tell who's receptive and who isn't. Most importantly though remember the worst that could happen is they ignore or don't want to talk to you.

Well, think about it: do you like being interrupted by strangers? Realistically, you lose nothing by approaching them anyway, but don't expect any kind of results with people who are occupied.

one told me to leave her alone, she was on her phone and had earphones on. You def need to know how to approach in cases like these.

I don't like interrupting people. Obviously not a good opener and somewhat disrespectful. Go to a bar instead. I'm not saying you can only approach at social events but if you're to do it in the middle of the day you gotta select your target very carefully. I personally HATE when someone wastes my precious free time that I might be using to read that book I've been wanting to read or listening to that new album that came out. All you gotta remember is that if she's open for business you will know. A good way to determine this is eye contact.
