What has been working for me every time so far in a bar scene is placing my hand on her lower back when approaching her. Almost talk to her from the side of her face for the moment. Its a smooth move because of the loud environment and it comes off almost natural. Do this periodically to establish that dynamic you're looking for.

stay in the moment helps a lot to find an oppurtunity. Once was with a girl who was pretty clear that it takes her awhile to warm up, but something dropped on the floor and I had a chance to tug her leg. She shrieked, but it was funny and it kept going a little bit more and more. that was a decision that took one second.

Just yesterday I read the ultimate kino guide that /u/CaliBboy linked ,before I went on a date. It actually comes pretty naturally, just the start is a bit awkward when you are not experienced. For example - Say you are mocking a girl at first, give her a light jab on the shoulder, that's kino. You agree on something? High five! You make some kind of a bet/deal - handshake! Look for chances to give her a hug to escalate (What I've done). After that you can escalate however you want if she's complying (not backing off or something,reaction to kino is the ultimate giveaway she's interested/not interested ).

Basically ,read the guide and the escalation ladder ,start with what I've told you (also back pats/hair touching) ,and the rest should come naturally (all based on my experience yesterday, but I'm pretty confident it would work every time).
