Well, how would you react if someone forgot your name ? If you like that person, you'd probably let that go right? and if you don't , you might hold it against them . Same would be the case with her. Talk to her , see how she feels about it. Apologize, but sincerely, not just because you want to sleep with her. See what happens .

I only see two and a half mistakes in this: The half: You went to the movies with her, even though you managed to get physical with her. One: You tried to kiss her at the end of the movie, not in the middle, at the start whatever. Two: You tried again at the end of the date. Those are all classic mistakes, which put much pressure on the both of you, and make it awkward for her. She knew that you would try it, and i bet she hoped you would not. Not because she doesn't want to be kissed by you, but just because it is hammered into the brains of people that those advances are just cringeworthy, mostly depicted by hollywood movies.

That you didn't say her right name was a minor fault, and if she would hold this against you, you would know that you just dodged a bullet there. Anways, i don't see this as a complete disaster, just as a date that was not perfect, which is not a big deal, and if she is grown up enough, and not totally repelled by you, i am sure she will want to go on another date with you, even though (or better said, because!) you made your intentions clear and made your move.

Your game is boring. Way too predictable. Women dont like it when they know exactly what you are going to do and exactly what you are thinking. Im pretty sure she knew you are planning on kissing her because your body langage started tensing up. A kiss needs to be spontaneous, when you feel it, go for it or you totally miss the genuine effect. You have to take risks.
