I was recently in a car accident in which my car was totaled. I miss the car but more importantly I can't afford to replace it. Thanks to my current support obligation vs the amount of money I actually earned I have been steadily accruing more and more debt each month despite paying. It's a long story but this is what is happening.

The nearest court building is not close enough to walk to and I have -0- money so I cannot take a cab. I have no friends in this State. Which is sort of why I moved here.

The form I need is, I believe, called "Motion to Modify Support Order". I had one mailed to me by the State but they sent it to an unoccupied neighbors house and I did not receive it until after neighbors moved in, months later. The form stated that it had to be returned within 3 days or I would forfeit the opportunity for modification for another 3 years.

I have been told by a few people that this is false and that I should just go ahead and fill the form out.

Problem is... I can't find it. I probably threw it out based on believing I had forfeited my right by not sending it in within 3 days.

I found the exact same form online once, and thought I had bookmarked the page. Alas, I did not. To make matters worse I simply cannot find the form now. All my Google-fu results in are instructions telling me to fill out the form but providing no links or methods of obtaining it other than going to the courthouse.

Any suggestions would be awesome, a link to the form or a friendly offer to mail me one would be awesomer. Yes, awesomer.
