This may belong in a sub like /r/getdisciplined but I feel like I can trust you guys to give it to me straight so I'm starting here. TL;DR at the bottom.

Guys, I could use some advice. All of my young adult life I was a complete beta. Not good at sports in high school, went through college with a few long-term steady relationships that just somehow fell in my lap, and then ended up in a bad marriage in my 30's. I'm 38 now and I haven't been laid in 2 years since my ex and I split up.

I've been reading PUA forums and TRP themed blogs for several years now. I feel like I have a good handle on what it takes to become alpha and attractive. I know I need to get in shape (I'm 40 lbs overweight), get wealthier (just getting back on my feet after an expensive divorce) and improve my social skills. (Natural introvert with a heavy dose of social anxiety.)

The problem is, I only seem to have the drive and discipline to go down a path of change for a few weeks before I default back to being lazy. I'll get psyched up, put together lists of goals for becoming my best self, march down a disciplined path of gym, studying, writing business plans for side income, and improving my diet. But once the initial enthusiasm wears off, I feel a strong pull to just surf reddit and watch TV every night after work. This pattern has repeated since I was in my teens and I really don't know how to break it.

Last night I literally watched 3 hours of television instead of doing homework for my masters degree. While I was sitting on the couch I repeatedly said to myself (outloud) "what are you doing?? You have stuff to do! Your goals are not being accomplished!! Get off your ass and do something that will improve your future self!!!."

And then I kept watching TV. What causes this behavior? Has anyone successfully found a way to permanently change your default behavior to one of action instead of apathy?

tl;dr: My default mindset is to act as if I have no goals or responsibilities. I tend to only do the bare minimum to get by except for occasional bursts of discipline and motivation. I want advice to change this.
