I disagree with the people saying to say where you're from. If she doesn't remember you probably didn't make as good an impression as you think.

"Hey it's X, it was nice meeting you the other day, down to do Y on Tuesday?"

I think a day or 2 after is the best, early enough so she remembers it. Tell her who are you, where you met and preferably continue the conversation you had earlier unless you didn't start talking about anything interesting.

Great job btw! Let her know you're doing something fun soon and invite her to join you. If she turns it down, go anyway! Consider it a solo practice run a date, like a tutorial mode in a video game lol. That way you look like you know what you're doing when you actually do that activity with a girl. Whenever you hang out focus more on having fun and enjoying yourself instead of keeping a girl entertained. Fun happy people are great to be around. Distracted nervous people? Not so much unfortunately.

EDIT: as for date ideas, dinner and a movie is kind of cliche. You don't really get a lot of time to talk during a movie. Something active like bowling and mini golf would be more fun. You can have a picnic at the beach during sunset. There are classes for making pottery that have "date deals" where a couple will get a discount and quick lesson. Dinner is fine but grabbing something to eat should be a small part of a date and not the main attraction. Going to the Zoo or a Casino would be really fun but would also be much more expensive and probably better for someone you are "exclusive" and not just dating.

Like iplaysmitegame says, text day of or after. Make sure the date goes smooth, play the date out in your mind first if you have to.
