Jokes aside tho, most of the time people won't notice things you worry about. Even if a girl points something about your ears, she does it to see if your easily shaken by superficial shit. I say don't worry about it.

I agree with @wangoli. To add to this make it a part of your identity and own it. Start making girls laugh with your ears and we all know how much girls love to laugh. But do it more so to overcome the insecurity of your ears. And be genuine with the jokes. Look at Channing Tatum, will smith, etc. so many celebs with big ears but because they accepted their flaws, had genuine fun, they made it work for them. Make sure you're comfortable saying it and you really find the jokes funny. So what if the jokes don't get laughs at first or you're a bit uncomfortable at first. Jim Carey said he used to go on stage and fail bad but he kept doing it for practice and look at him now. Eventually you'll get comfortable the more you do it and you'll find yourself. Good luck!

I've a few issues with how I look, ears used to be one of them. A few years ago I decided they would be the most obvious and cost effective flaw that could be fixed. I am happy with the end result and made me a bit less critical about my features.

You know how many people noticed I had it done? One, and that was a guy I worked with. All my friends, who I'd never told about the procedure, never said anything.

If had an unlimited budget, I'd get other things fixed. Ultimately if you think it's going to ease your insecurities, I say go for it. Just don't expect some amazing response from others!

Are your ears REALLY sticking out or do you just think they are? Some people tend to have a real problem with perspective, overly focusing on a problem they may very well not have.

No, don't get them "pinned" or whatever. Only do that if your face is irredeemably bad otherwise. Because your good features will make up for the imperfect features.

I can think of any number of female celebrities who have ears like that, who I think are absolutely gorgeous anyway. Kristen Stewart and Kristen Ritter are a couple of examples for you. Maggie Q has strange facial features, but the look is so interesting, it's stunning.

The point is that if you don't mind the same flaws in women, it probably matters less than you think.
