Do something that he has never done, but that he would enjoy. Something like a Segway tour of your city (if you have them), or a trip to an indoor skydiving center.

Go-Karts, Shooting Range, Comic shop, (what ever his hobbies are) and get him something to remember the occasion.

Of course, depending on where you live, and what your relationship is like, nothing says "I love you" like the gift of another woman, a close friend or professional (whether it's a threesome with you, or a gift for him, and you watch or just something you can give him and you go do something else, would be up to you).

I always plan surprise vacations/outings for my wife and she loves the adventure I take her and the kids on.

She kept telling me someday she'd surprise me to which I would laugh in her face.

Well, one day she told me we were leaving on Friday and coming home on Sunday. I had no idea.

She booked us a tour of the Anheuser Busch Brewery, Indoor Skydiving, and Rock wall climbing it was a fucking awesome weekend.

I take it you're not asking for gift advice but more something like an experience?

If my wife were to give me an interesting experience as a present it could be skydiving, bungee jumping, climbing, a shooting range with interesting weapons or a tactical course or where I could shoot my rifle at 500+ yards, my wife demonstrating her newfound strip tease and lap dancing skills.
