Whatever the reason you don't approach is... It'll almost never be as bad as however bad you're making it out to be in your head if you approach. Most of the time, if anything, the girl you're approaching really appreciates and is attracted to just the confidence of you coming up to her and saying something in general.

Even if you can't say anything just being honest and conveying your emotions to her saying something like "sorry... I'm trying to think of something to say but you're just so beautiful I'm absolutely speechless right now... Could I just get your name? I'm blank." is way better than not making a move at all you know?

First thing is not to get harsh on yourself. You are creating unnecessary tension to act, when proper circumstances and vibe may not be there. Encourage yourself instead to make just that one litte step further. Smile at a girl, come little closer to her. Then you need to figure out some legit scenario, most of your anxiety comes from the simple fact that your plan is too vague. For example, come to her and say "Hi, I have spotted that you are really cute, and I want to meet you". Don't neglect your emotions, use them to figure out the proper way. If you didn't manage it this time, just wish yourself better luck next time.

I suggest you a smooth indirect approach but after a minute or so you can always make a transition to introduce yourself. It is more natural to talk like that to someone.
If you are under 20 maybe it doesn't matter otherwise it is awkward.
Imagine going to a park and talking to an old man, they will never start a conversation telling their name, they will talk for an hour about all kinds of things without knowing your name... the name doesn't matter unless you see that it is a nice vibe between you and the girl.

You need a shift in beliefs. Right now you have a belief that something may go wrong if you approach. Mostly, because you don't have alot of experience approaching.
You need to approach and train your mind that its not a big deal. You can only do that with experience approaching. So, start approaching these girls and see what's on the other side. I mean, did you think it was more complicated than that?
Adapt healthy mindsets so you aren't beating yourself up after an approach. Don't make a big deal about it. But, you need to practice approaching, its as simple as that. Focus on your inner game. Get to a point where approaching isn't a big deal anymore because your beliefs are in line with things going well. You only get there from practicing.

All men, especially when starting, feel just how you feel. With repetition it gets less over time but is never completely fine. The hardest, most nervous I’ve ever been was my VERY FIRST cold approach. The first one is the biggest roadblock, took me weeks of walking around Walmart to finally have the balls to do lol

Don't fret over this, uni libraries are not a good place to go in trying to pick up or meet strangers. People who go are usually there for a specific reason, and have something important they are doing. other places just literally open it up with conversation. You don't have to ask name or give yours like its formal, just make some conversation that is open to reciprocation. the spontaneity can work well, and you will learn don't think you need to act differently to some girls than others based off your attractiveness. Shit sorts itself out, even though that sounds kind of empty, but it is hard to explain. Dont compound your anxiety by thinking every eye contact is a missed relationship or something, it can make thing worse over time.

If that anxiety comes with unworthy feelings,or something like you are not enough, you might have inner child issues. Check if it's your case and work on it through meditations or hypnosis
Oh yeah I’ve got a bunch of different issues unrelated to this. I used to meditate daily for 2 years, fell off the train for a while but have been back on it for a bout a week now :)

I suffer from anxiety, had a diagnosis well over a decade ago. I found ways to manage it. Didn't stop me from graduating, excelling professionally or dating/relationships with lots of amazin chicks.
Exercise, do a sport, focus on your personal relationships and friendships, push yourself out of your comfort zone to achieve ur goals. Don't fixate on dating/women. It sounds cliché but just be the best you and you'll meet women who like you. Focusing on the other things will increase ur confidence. Honestly, it gets easier, focus on self-improvement.

Hypnosis. Go to a hypnotist and tell him your situation. Hypnosis is like anxieties Kryptonite, and it takes effect quickly. Much cheaper and more direct than talk therapy. Otherwise, you might try beta blockers if the situation is bad until you build natural confidence. Limit your anxiety ceiling.
