Thanks, this is useful. I'm pretty bad at keeping conversations going and handling silences. I'm going to keep #4 and #5 in mind. #3 is hard for me, though --- how do I make it feel like I'm not just making boring small talk? Whenever I ask a question out of the blue I feel like I'm coming off as desperately searching for something to say. What are some techniques for making that feel more natural?

If the girl I'm talking to has been investing in the conversation and giving me IOI's, I usually say "I'm curious about you.... [instert random question about her]". It shows your interested in her and basically gives you a reason why your asking. Its OK to show a little interest in a woman you've been talking to, flirting with, touching, making her laugh etc. Just make sure she is showing you interest first.
Reward her every now and then by saying things "I'm curious about you... [random question about her]" and "You're interesting. Tell me... [insert random question about her]". Just make sure to smile when you say this and ask like you are genuinely interested in what she has to say and it will seem natural.
Obviously, you should NEVER display too much interest (i.e. showing her more interest than she is showing you.) Keep up the push/pull.

Moments of silence don't have to be a bad thing. The key is to OWN the silence. It's not an "awkward moment" it's just a moment when you're both being silent, thinking. Just look at her and smile a warm friendly smile... and don't say a word.
I'm having such a great time right now, you’re fun :)"
Exactly. But don't be afraid to allow the silence to linger for a while before doing this. Those silent moments can be good for creating tension, which is a good thing.
Also, make sure to allow enough silent time for her to start looking for a way to fill the silence. Don't feel that you have to fill every moment of the conversation.
And you can always go "cocky/funny" as well, if you want to:
"So... do I have to carry this entire conversation, or are you gonna say something?" <grin>
"Cat got your tongue, or are you just intimidated by my stunning good looks?" <grin>
etc. etc.
