No, i recently visited a friend in NY, who is a successful indie movie editor. She had her phone out constantly as she was working, attempting to make sure everything was going to be okay with her associates in S.Korea. She was hot, smart, and funny... but im not going out with her again.

at no point that is ok. I have literally walked out on a date at that very second when she texted someone. she ended up apologizing to me many times and has been a fuck buddy since.

I wouldn't take my phone out. If you think it'll constantly be going off, turn the phone off/on silent. If you must check the score (big sports fan myself, so I could understand) just excuse yourself head to the bathroom & check up on the scores. I wouldn't do this more than once though.

Once or twice is okay, anything more than that unless an emergency is a huge no. It won't dhv. If a girl is one of those texting whores, i simply ask the girl if i can see her phone, once she hands it over to me put it in my pocket and smile at her.

Never ever. Only exception being if you are pulling the phone out to show photos of some cool shit you've done, show some video or something very relevant to the conversation. Make sure it's GOLDEN though. Otherwise, just keep it tucked away.

No, no it is not. This makes your dinner friend not important enough to be around. Might as well not be at dinner with them if you are willing to engage conversation with others through phone or otherwise.

It would be rude.
