Good question - you pretty want to initiate/escalate kino at emotional high-points. So if you're talking to a girl and you both happen to like the same movie: "No way, you like that movie too?! That's awesome, give me a high-five." Same thing with a hug, but depending on the situation it's usually better to start off slow - unless you're getting IOIs. If you're getting IOIs you can obviously escalate faster - if she came up to you in the last example you could easily replace that high-five with the hug.

Another example would be if she said something really funny: "Oh my god you're hilarious - give me a hug." You're essentially linking her positive emotional state with your touch. So hopefully at some point - she forgets about what was actually making her feel good and deduces that it was actually your touch that made her feel good when in reality is wasn't

I usually hug girls when I'm leaving them or when I'm seeing them again for the second time. I have also started to implemented two kisses on the cheeks a la European style and it hasn't backfired yet.

Hugs can always be appropriate especially a bar-like environment; totally okay to hand out hugs! (That may be a little different if it's a co-worker or something).

hugs are pretty innocuous, and few girls would be opposed to one no matter what the circumstance or how unattractive you are. basically, it's a more affectionate version of a handshake, because shaking a girl's hand after meeting her is just old-fashioned and awkward. instances when the hug is appropriate:

when you casually bump into a girl who you like / girl friend without it being planned (happens a lot if you're in college)
when you are leaving and telling a girl you like / girl friend bye (only if you didn't hug her at the open)
when you are telling a girl you just met bye
when you have friendzoned a girl on account of her not making the cut, being obese, being emotional, etc.

You can do a hug almost any time. The most important place to do it is when you're leaving. I also like to announce a hug, so she knows where it's coming from.

I sit down on a bench next to a girl in the park. I start talking with her. After a ten minute conversation, I tell her I have to leave. I exchange contact info with her.

Then I stand up. But before I walk off, I say, "Stand up. Give me a hug." Sometimes she has things in her lap, like a book and an ipod that she has to shuffle around before she stands up and gives me a hug.

Once in a while, particularly if it wasn't as great an interaction, she'll decline the hug and say something like, "I don't hug strangers," or something like that, although I've had interactions where she declines the hug but still contacts me later (I give her my contact info instead of taking hers).

Hugs are pretty much always appropriate. You only feel awkward because it's not a habit for you.

I generally hug girls when I meet them. If I'm not feeling up for it (as in, I'm tired or something, and I feel too awkward for a hug), I at least go for a high five.

In my opinion there's several types of hugs. Even if you aren't comfortable doing a full-frontal embrace, you can still do an angled, one-handed hug. It's slightly less personal than the alternative and still gets you in there.
