Sounds like you already have diagnosed the problems and the causes. There are a ton of solutions for your problems:

Low self-esteem → Improve Confidence / SMV (lifting, dressing better, outcome independece, etc.)
Oneitis → Abundance mentality, don't let her get into your head, etc.
Being a Nice Guy → I'm pretty sure you could find tons of information on both here as well as the TRP subreddit (in particular the book 'No More Mr. Nice Guy').

I have a feeling you already know the answers to your problems, but are just not willing to face them. Either way, it's all on the side bar.

You can slowly lose this girl by feeding her increasing amounts of validation and non-sexual attention because you're afraid of losing her, or you can withdraw those things and either lose her right away or fix the dynamic of the relationship for the long-term.

If she ends it, you were going to lose her anyway, and all you would have been doing is lying to yourself and lowering your self-respect for a few weeks.
