I know what I'm about to say may be a bit forward considering I don't know any more context than you provided. If you are doing everything that could be reasonably expected of you as a boyfriend, which I believe you do as you are still trying to make it work, then you can ask her why doesn't she feel the need to work on herself in this relationship. If she is expecting unconditional with equal to or less than bare minimal effort to make herself attractive to the man she loves, then she doesn't deserve that man.

A partner is someone you should be willing to contend with almost as a guiding compass that helps you as you help them become better people. If you become stagnant in parts of your life, that part of you is likely to die someday. Maybe she has some inner demons that prevent her from wanting to love herself and this is her trying to push you, a good man away subconsciously. Ask yourself while being as honest as you can if you deserve this kind of treatment as a lover and as the person you truly are. You should know how you feel and decide how to go about this. All begins with a conversation. Make a plan with her how you two are going to untangle your finances. Who's moving, who gets what, etc.

Then a year from now when she works out and gets better looking than when you first met her do not cave back in for it, because it's only temporary.

Then just some practical things. You ending it is going to bring out the BItch you have never seen. If she has access to anything of yours that is valuable remove it now and hide it. Any financial information, change it. Any passwords for anything of consequence change it.
