If the ex has any partial custody of the kid, they can just march down to the Social Security office themselves. What's the problem here?

That person needs to get a court order that the other parent cannot take the child out of the country. It might be possible for the court to demand she turn over her own passport to the clerk. If she's not trying to do take the child out of country then she shouldn't have a reason to argue against it. Without a court order it will be hard to keep her from taking the kid out of the country.

No, you can just order a replacement card with just the birth cert(no need for the number). Also, if you have then just be a good guy and give them to the ex, this is the type of small fights judges don't want to deal with.
