Meditation and incrementally increased exposure.

That's the biggest and best way to deal with it.

You can find meditation help anywhere, incrementally increased exposure is just that, go out a little more every week until it's daily and comfortable. Always stay slightly longer than your comfort zone. Always talk to someone slightly more unfamiliar. Just keep moving it up, like adding plates at the gym. Eventually you'll be much more comfortable.

Mindfulness and increased exposure or "exposure therapy" are helpful tools for dealing with SA. But, if it's more pathological as in it's a legit anxiety disorder or rooted in some trauma then those tools might not be quite enough.

Any idea what might be at the root of it?

That's the problem.

It's not appearance, it's your mind you need to work on.

Google "meditation" and read everything you can. Then, make this a priority. Study Zen related topics, relax, and stop caring what people think of you.

I have suffered from this. Actually, still a bit from time to time, but very rarely.

Meditation is a must. Therapy might also help.

Another thing I want to say is; just read about Stoicism and apply it. Any mental illness (I know it's a harsh word but it's true) is a synonym for irrationality. Your best bet would be to practise rationality.

Just equalize the "real" reality to "your" reality. You will be a winner in a short time. But, it takes work. Real work.

I had the same thing growing up. You care too much about what other people think about you. Try visiting /r/howtonotgiveafuck and just have some confidence in who you really are. Might lose some friends, might make some new ones. It's all for the best
