When I cold approach I make a point of telling the truth, even if it means not getting the date or whatever. I find that it works insanely well. Women LOVE when a guy is just honest with them for once, instead of hiding who they really are in a bid to come off as manly. It's a huge relief to them and they love you for it.

It all depends on the conversation. I just tell the truth.
If they asked if I had an STD and I did I'd tell them I did.
Do drugs? sure
Just looking to get laid? yep
not that any of them have asked things like this on the first approach
I think where this shines most is with insecurity and emotions. When you can open up from a place of confidence they find that very very enduring. Like I sat down with this one gal, and we got to talking about my ex (we were talking for a long time at this point), she asked if I still missed her etc. and I answered honestly that I did miss her alot, she asked if I'd cried about her at all and I said that I did, usually thinking about how much I missed her. Next thing I knew she wouldn't stop talking, telling me how she wishes that more men were open and honest like me and how its soooo saught after by women nowadays and their basically starving for it (we ended up getting together acouple days after this too).
It just all has to do with delivery. If you own it in your mind, are confident and are coming from a place of strength, sharing your feelings and the truth is incredibly attractive to women. It's when you can't help but share your feelings that things go south.
I should also add that I let the girl steer the conversation in that direction. Its just when it comes up I don't shy away from it

Yes, would helpful to know what they like him being honest about. I suspect it's not sharing information about that thing he's planning to see the doctor for.

You don't tell her you like her, that just gives her validation and it cheapens your value. But you also have to play who you are naturally.

I think you have to play yourself naturally. If you're not naturally a vulnerable or emotional person, faking emotions is going to come off as strange.
(this is not to brag)
Example; I am 6'3", 250ish, heavily tattooed, beard, broad-shouldered, I've been in boxing and muay thai for years, was a bouncer blah blah blah. I've been told I walk around like I own the place but it's just because very few people try to start with me so I'm usually very relaxed. I'm also not very emotional, (hard childhood, nothing really surprises me anymore).
There are some women who are just not into me, they don't like my look at all. There are other women, that LOVE my look. They want to date the big scary guy, that's who they're into (I'm cool with that), these women are not into me talking about my feelings, they want the strong silent type. They don't like men who complain or cry at movies or whatever. That's not their style.
Now, what wouldn't work with these women is if I was trying to fake that I'm some big scary tough guy but I was secretly needy and cried at the drop of a hat. That would totally gross them out because I was being disingenous to myself.
All that being said, If you're an empathic guy who deeply cares about the well-being of humanity and animals, you would be doing yourself a HUGE disservice to the world by trying to act like someone like me.
