Ask to use the bathroom.

If, once you get up there, she seems comfortable with the idea of you being there, she was probably just looking for a plausible reason to invite you upstairs in the first place.

BTW, if she says no, shes tired or some other crap, let her know that you atleast want your goodnight kiss and kiss her. She might change her mind, and if she refuses the kiss, she will most likely not want to go out with you again anyway. You have now successfully established where you are with this girl.

If all else fails, and she doesn't invite you in for a bag of popcorn, you can take of your clothes and do naked man in the street outside her window. THEN, and only then, will you be 100% sure if she is gonna change her mind.

Try to establish yourself as the leader in the date first by choosing where you go, where you sit, what time. She should feel comfortable with you dictating how things go and just following along. Then you could say something like "your place is nearby, lets go there." At a date a girl mentioned to me that she had a large vinyl collection, so I just said "Show me, Lets go right now" and put my drink down. On the way there I said something in the form of "I cant stay very long, I have work tomorrow" because I noticed she tensed up some. As long as they are used and comfortable with you calling the shots then you shouldn't have a problem suggesting going to their place.

For your place have her come pick you up and go someplace by you(you can say it makes more sense instead of going back and forth to pick her up). Invite her in for a sec while you grab your coat or something then head out. By her being in your home, even for a minute or two, it places far less tension for at the end of the date since shes already seen it and knows you have a cool friendly place to be. At the end just invite her back to watch a movie or something and take it from there.

When does this scenario ever occur? A night goes fantastic, you pull this girl, you are into her, she is into you. The moment you say "let's go", the only possible place to go, is her place, she lets you walk her home. Honestly, you are already in her house the moment she agrees to leave, either that or you have been misreading her the entire evening and she is in fact a devout christian who does not want strange men in her home. If that happens to you, you need to step it up senpai, and spend a tad more time getting to know the girl.

Early in the date, talk about her apartment. Ask her where she lives and if she lives close be like "I bet you've decorated your apartment really cool, interesting, funky." And she will of course agree (because girls are into decorating like that), and then you just say "Ok well you should invite me to see it sometime." But when you say that immediately change the subject to leave that thought dangling in her head. By the end of the date, ask her if she'd like you to walk her home and then bring up the "are you going to invite me to see your apartment" line again.
