If you’re going to therapy and it’s not getting better. That’s scary stuff. Not sure what us strangers can do.
Baby steps man. Maybe start with letting her see you in your underwear.

If that doesn’t work and it doesn’t work out you should consider dating someone asexual.
Other then just “getting over it” you could date someone who wants to be emotionally connected to you but is totally okay with your desire to not be naked.

This sounds like an ego issue? Your ego is supposed to help protect you but it can also prevent you from actually living and having profoundly important experiences if you let it take control. You have to hold the reins and confront your fears instead of trying to run away from them. Your fear of being laughed at or perceived as inadequate is crippling you.
You need to have a "momento mori" epiphany and remind yourself it doesn't matter if you're well endowed or not, beautiful or not, etc. Especially to yourself, that's narcissism. This life is all temporary. One day this vessel you currently inhabit will decompose back into the universe.
It's not real, it's just an illusion and a very temporary one at that. Seize the time you have with this illusion while you can as if you end up on your death bed in the future, you'll regret what you didn't experience far more than anything you fear now, especially rejection.
Embrace rejection, it is the only thing that makes the experience of acceptance real and worthwhile.

You’re not alone in this concern but that’s the perfectionist in you… one, the fact that you have a girlfriend should resonate the thought that someone is attracted to you… two, tell you that she’s see’s your imperfections and sounds like she’s clearly still turned on. Some people start with candlelight romance but if not cut all the lights off and start there! There’s a difference between f-in and making love! Makin love is more than the point of penetration… it’s about the exchange of chemistry… the physical expression of how much you mean to that person. Take care of her the exact same way you listed above and when it comes time to penetrate, she’ll be on a natural high and you both will be satisfied. Don’t be afraid to play some nice romantic music, hit the lights and let it happen. Also… shots work too… especially tequila. It will calm your nerves. Best of luck in your next steps

Really sit down and think what is causing this.
It can be anything from religious reasons, body insecurities, etc. Really think it through.
Then, try to come up with what you'd be comfortable with and communicate that to your gf
She seems to be into you enough to stay for now. Losing her is not the end of the world despite your brain telling you it is.
