You don't. You just have to lead and dance with her. For example, playfully grab her hand(s) and initiate dancing.

EDIT: The reply below is absolutely right; YOU NEED TO BE HAVING A GREAT TIME EVEN BEFORE you think about approaching a girl and I thought I'd write in bit more detail. Think of it as you are the fucking party in that moment on the dancefloor and your job is to share your own happiness & positivity to the people around you; not just women. Once you are high on life, make eye contact with your girl of interest if possible, otherwise start sharing your happiness andby either inviting her to dance with you by offering your hand OR just playfully and flirtatiously grasping her hand(s) and dance away. Fuck what happens next, as long as you are having a great time if anything happens next is an added bonus.

Ask her non verbally. Signal her to go to the dance floor with you. It's not a big deal if you don't make it a big deal. You can even grab the girl from a group of friends. It's not an issue, you just want a dance, what she is above you? People don't like to say no. If she says "no", you are back to where you already were, nothing lost.

Great answers already here, I'm mostly just elaborating on them:

getting in the correct state of mind: you can warm up socially by having a chat with anyone and everyone. There's a guard at the entrance? Chat him up. Girls at the cloakroom? Definitely chat them up, tell them a lighthearted joke, they are BORED. Waiting at the bar? Chat the other people up. This exercise alleviates approach anxiety, and it will make approaching girls for a dance normal and even logical: we are here to have fun, dancing is fun, so we dance.
Watch for the IOIs. When you are opened up socially, girls will notice that. If you look like an approachable, decent looking guy, you will have eye conctact from girls who want you to approach them. Remember them.
Don't put all your eggs in one basket, if you find a girl showing IOIs, you don't need to rush to her immediately and cling on her for the rest of the night. Acknowledge them with a smile, a wave, or anything you find appropriate given the situation, distance, degrees of freedom, etc, and go on with any interactions you find natural at that moment. If it happens to be that particular girl, great. The reason for this is that if you are flying through the entire place to that girl, and it takes 10-20 seconds, it is long enough for approach anxiety to build up. If you are just going with the flow, chatting up anyone around you, you don't have enough time to get anxious, you are already in the middle of the interaction.
When a girl is into you, its very easy, you don't need to complicate it at all. Just be calm, confident, and if you take her hand, she will follow you: it is exactly what she wants. No need for awkward chit-chat at that point if you want to dance with her.
On dancing lessons: they are a great help if you need confidence. If you feel you know what you are doing on the dancefloor, that confidence will ease girls into dancing with you. I can't dance by most definitions, yet, when I noticed a girl and her IOIs, I just reached out my hand, she took it: it wasn't even a dance, it was foreplay, mind numbing and goosebumpy. If you can't get this comfortable with an unknown girl while friends and anonymous people are watching you, dance lessons are great practice in a controlled environment.

I usually initiate by coming to the girl i like the most, grabbing her arm gently to get attention and introducing myself. Works very well for me, like 70% success rate, but yeah i look pretty good.
