For the first stage of the surgery, the doctor uses a steel instrument to stretch apart the pinpoint opening of the baby's penis. Then he thrusts that instrument into the widened penis opening and rips apart the membranes that connect the foreskin to the head of the penis. This ripping sound is audible to people in the room, even above the baby's screams.

Once the doctor has ripped the foreskin away from the glans, he clamps the foreskin lengthwise with an instrument, to crush the blood vessels (in order to minimize bleeding) and to create the "crush line" for the initial cut in the baby's foreskin.

Next the doctor uses scissors to split open the foreskin along the crush line. He peels the separated foreskin back from the head of the penis, places the Gomco clamp bell over the glans (to protect it during the amputation), pulls the split foreskin over the bell, and safety pins the two pieces together.

Now he pulls the foreskin and bell through the bottom plate of the Gomco clamp, adjusts the contraption for the amount of penis he wants to cut off (an arbitrary decision), and tightens down the Gomco clamp screw.

The doctor waits a few minutes at this point for the tremendous pressure of the Gomco clamp to crush the blood vessels to the foreskin, to minimize the risk of bleeding. Once he is satisfied, the doctor uses a scalpel to cut around the bell and amputate the crushed foreskin from the penis.

Finally the doctor disassembles the Gomco clamp, being careful to separate its lower part from the remaining shaft skin because the tremendous crushing pressure makes the steel device stick to the baby's wounded penis.

The doctor now wraps the inflamed penis in gauze with gobs of Vaseline to prevent the penis from sticking to the diaper and/or getting urine and feces in the wound.

The amputated foreskin remains on the rod assembly of the Gomco clamp. The medical provider may preserve the foreskin for resale to an industrial company that uses it for cosmetics & pharmaceutical products.

The baby is now circumcised for life. The most erogenous structure on his body has been destroyed, leaving him a workable penis for urination and reproduction. This is the barbaric surgery that American doctors have sold to naive parents for 100 years.
