I see where you're going with this, but it's a poor parallel due to the way that society handles actual pregnancy vs falsely claimed pregnancy:

A woman who falsely claims pregnancy to a partner faces, at most, the loss of that partner if he discovers her dishonesty.
A man who uses this paper to achieve unprotected sex with a woman faces possible child support for years after.
A man who uses this paper during paternity proceedings faces the likelihood of court ordered DNA testing, rendering this paper useless and potentially criminal charges for filing of false evidence.

Well here's what it looks like. So just get out the good old photoshop or use word to generate your own from scratch. Copy and past some fancy looking signature in there, etc and you're set. Just print it out, fold it up and keep it in some unmarked envelope.

You can even have two papers, one that shows a vasectomy taking place 6 months earlier and it could be some kind of medical receipt.

Easier than that. When you produce a sperm sample, you do it in private. Just take one into the cubicle which has already been microwaved.
