This is a cry for validation, reassurance, and comfort. I also consider it to be a very inappropriate question for any woman worth her salt to ask. Why? Because it shows she either hasn't been, or doesn't know how to recognize the different ways men display approval, favor, generosity, and love. Asking that question is the equivalent of fishing for compliments. Well rounded, stable adults don't need to corner their loved ones in order to feel better.

As for how to respond:

If she's a plate - "Who says I do?" or you can tease her.
If she's your SO - "Because you know better than to ask pointless questions" (tone and your ability to come off as charming will determine how successfully you can say something like that).

Very context dependent. Play it by ear. I'd tend to favor something like "Don't think so lowly of yourself, it's unattractive," if this just came out of nowhere. But if I could tell she was genuinely having a very bad day (like she ran over and killed a puppy) and needed some comfort to ease her insecurities, I'd be much more sensitive.

Most if the time a girl will ask this for two reasons, depending on context: either her self esteem is suffering and she's wondering why you would love someone like her, or she wants attention and wants to hear you gush about her. Either way it's not good, but in the former case address why she feels so worthless, in the latter probably a soft neg to bring her back to earth.
