Hey bro, I was about 280 at 23 years old and most docs wanted to not put me on TRT which was annoying, but one put me on clomophine and some vitamin supplements and my level went up to 750 within about 6 months or so. Definitely see a hormone doctor or endocrine.

Your level varies throughout the day peaking in the morning, and yes, stress, sleep and diet will affect it too. Blood tests will also be more accurate than saliva tests.

Try to reduce stress and improve sleep quality with meditation, and exercise. If it's possible, try to get tested again after a few relaxed days, and in the morning.

If it's still low, see a doctor.

I'm going to tag on to what's already been said:

Good quality sleep is huge. Do what you need to get there: meditation, sleep hygiene, ZMA. If generally about melatonin except for occasional use. Wearing a good high in potassium a couple of hours before bed can help alot too (think avocados or something, not bananas- to much sugar). Avoid alcohol with a couple hours before bed at least, and in general will screw up your hormones.

Stress can't be avoided and can cause problems with your hormones even if you are taking TRT, so my advice there is to figure out a way you can manage it. You can find all sorts of advice on that but it needs to be what works for you.

Because your levels can fluctuate so much, one test doesn't really mean much. Especially in a guy your age they should do three T tests before they put you on TRT. I once was worried myself. Test #1: 260s. One week later it was 600s and I don't recall making any changes. I was 25 at the time.

Lifting: big compound movements will get you maximum results. I recommend reading a ton. You don't need to be on the gym 6 days a week to get great results and that might even be counterproductive to us amateurs. My favorite is always the Texas method (mark rippetoe). That being said, you need to be eating enough to recover sufficiently or else your cortisol and estrogen levels will sky rocket. Also, reducing body fat can make a big difference, and so will adding muscle.

Saturated fat is huge for T production. Not advocating eating a rib eye every night, but animal fat is going to help, like has already been said.

Low vitamin D levels can be a problem, too, and most of us don't get nearly as much sun as we think even in lower latitudes. FDA says 400IU daily, but you can easily take 1000-2000IU daily without any much risk.

I would say, before you see an endocrinologist, make sure your doing everything you can to benefit yourself because do you really want to be poking yourself in the ass long term? If shit's not changing, yeah, see a doctor but even then, low T might be a symptom and not the root cause of your problems. Good luck!

One thing you did not mention about your diet; what's your macro ratio? Specifically, how many percent of your daily calories come from fat? The healthy range would be at least 20-30% because fat is essential for hormone production.

Furthermore, I suppose it's been said countless times already but high stress is harmful to our testosterone levels. Our body produces higher amount of the cortisol which lower our testosterone levels. It gets worse with chronic stress when the stressors remain for an extended period of time. Studies have shown that meditation helps to a certain extend (for stress).

Lastly, lack of sleep also increases cortisol levels.
