I’ve been on TRT since I was 24. I abused steroids since I was 19 and fucked up my endocrine system. Your current levels are low for sure. Although understand this is a life long commitment unless you want to have low T for the rest of your life. I assure you once you start and just randomly stop let’s say when you’re 25, your body will be fucked beyond repair and your levels will not return to what they are now. You will be dependent on this to have good energy levels, gains in the gym, sex drive and a working dick. You stop, you’re fucked. Consider this before joining the dark side.

Lift for 6 months and then go back. See what your t levels are after some consistent lifting.

You are a smart young man. I love how you know about cortisol and about your hormones, that in fact is something great and I encourage you to continue.

About the TRT, most doctors will find it hard to give TRT to a young man, even if he has low testosterone. It is not imposible but hard. This doesnt not mean you are wrong, hell, you are right and I love how you are seeking improvement.

You are on the right path, but please dont be disappointed if they deny you it at first. Keep on your path and eventually you will find someone that will fullfil all you want. Plus I like how you are doing the reading for yourself, big respect brother.

At your age, when you're supposed to be a young buck, I'm willing to be 9/10 that your problem is fully dietary. One you take TRT, your body's endogenous (internal) production will drop and you will be on it for life. You do not want this.

Here is what I would do. First stop drinking or consuming all dairy (milk, cheese, yogurt). Dairy since the 1970s is full of estrogens since a good portion of it comes from pregnant cows (genetic modification from several generations back).

You might also want to drop your meat intake. Atherosclerosis starts young and it block blood vessels, especially small ones first like what feeds your dick and your balls and glands. Your balls and your adrenals (sit on top of the kidneys) are the biggest source of testosterone in your body.

If you're drinking beer, reduce that too. The hops have lots of a potent phytoestrogen and women who used to harvest that stuff were effected.

Have a source of saturated fat, some nuts for your nuts is the healthiest source. This helps with Testosterone production. But supplement less than 100% a day. Saturated fat also promotes atherosclerosis -- so it's a balancing act.

Eat PLENTY of greens. Arugula and brocolli and a variety of other stuff. Greens do all types of healthy things, but arugula especially has nitric oxide which does wonders for vascular health.

Get off fast food and fried food. Atherosclerosis again. Stay away from processed vegetable oils (look at ingredients list). Vegetable oils in it's natural packaging is safe. Eat real food, as close to grown as possible. Ie oatmeal not granola bars. Corn on the cob, not corn chips.

Make sure you have an iodine source. The easiest way is a few pinches of iodized salt daily. A lot of processed food has a lot of salt but that salt isn't iodized. Because of this and the soil is depleted, lots of people have some level of iodine deficiency.

Iodine helps your thyroid. Your thyroid is the master gland, which controls the other glands. Do NOT exceed 100% iodine. Too much iodine will suppress your thyroid just like too little will. Shoot for 2/3s RDA supplementation. If your diet is healthy, the rest will be gotten there. A little higher than RDA through diet is okay, but don't go around getting 500% daily and then plop natural dietary sources on top.

If you wanna do TRT, then do it but don't use your current low levels as an excuse / justification. There is many things that can influence it which vary daily (sleep or lack thereof can have a huge impact) and there are things you can do to up your T levels.

Downsides? Use Google. It's not new or rocket science.

What can you do to up your T levels:

diet (reduce sugar to as low as possible, more fat which you get when replacing sugar with protein/meat)
lower body fat
sleep more (8 hrs a day should be the goal)
less porn and masturbating

Also you might already be adjusting upwards but it will take time. Maybe redo test in 6 month after improving more on above points.

Note: I was pretty much at the same level about 5 years ago and my newest result had me at >500ng/dl. There was one between around 420 so steady upwards trend but it just takes a lot of time. (I'm much older than you so T rising is actually surprising thing and it should go much faster at your age).

tl:dr: Have patience. If you continue improving this will improve as well. Or if you want to do roids, inform yourself (google, other subreddits,...) and do it. No need to have an "excuse" to do it.

Testosterone is the one area of medicine biased against treatment and patient well-being. Many people, including many docs, think so what and snicker about it. Situations like this are why it can indeed be a big deal. It sounds like you may have pituitary disorder. The prior case of gyno indicates your test/estrogen levels were out of whack and probably still are. You might need to find a new doc, as she sounds like an idiot. The testosterone ranges physicians use are reflective of the bias against treatment. These ranges are not for levels that are optimal, average, or average by age, but the undefined label "normal," and the ranges are done this way to preclude treatment. Your testosterone level is less than half of what it should be at your age but because of the ranges being used and the dismissive attitudes of many physicians toward this issue, you were told "everything is fine." The gyno coupled with that low of a level should have been red flags to her.

TRT shouldn't be your first step. You should get an MRI of your pituitary. A course of treatment of hgh or HCG could kickstart your natural production and get your hormone levels to where they should be. Note I said course of treatment. This wouldn't be for life. More like 6 months or a year. But the appropriate treatment really depends on further testing.

Lifestyle changes are good for overall health, but they won't significantly increase your testosterone levels. You have a medical issue and should get it treated.

So if you're still losing weight, the t level may drop. If you're still losing weight, get tested again after you are done losing weight and have been maintaining that weight with a good amount of food for a while

My man just wanted to offer my two cents. I wouldn't go straight to the TRT as it seems like a quick fix.

You already know about hormones now just learn more. For example sleep in a pitch black room with eye ask and ear plugs with brown noise in the background to get best recovery possible.

You're also never going to reach your potential if you're not training hard in the gym.

Also take a look at the KETO diet, it's great for resetting your hormals to their optimal level.

Also you'll need to cut gluten from your life, it's filled with estrogen

Sure they are on the low side of the physiological range but that’s not a reason to get TRT. TRT is meant for ppl who have consistently lower than normal testosterone. Below 300 consistently! You had one test above 300 so I don’t consider your case to be a case for TRT. Your 18 and you don’t need to wreck your body so young. Just relax keep lifting and eat right and things will go your way.
