Given the fact that you guys have seen each other more than once before and it isn't a random girl on the street, I would preface your approach acknowledging that you've made eye contact a few times before with something like "hey I've seen you around here a few times and I wanted to come say hi because I thought you were super cute".

After she responds, I'd recommend having a little small talk to build comfort (ask her name, where she's from, what she's up to) and then tell her "we should go out sometime, I know this really great bar we can check out".

If it's just 1 specific girl you like at the gym the odds are very against you. It's very rare she'll say yes. Without trying to get to know a girl before asking her out has proven time and time again to fail. It always will. "Hey, I thought I'd say hello since I've seen you a few times here. I'm bob, what's your name- I'd like to go out for a coffee if you're free this weekend!?

If she says, “but I don’t even know you” you say:

“I’m a lot like that guy Joe from You (Netflix show)” and smile.

She will laugh and then you smile and say how about you give me your number and we can hang out sometime.

Beginning a conversation with a request for a date is awkward. You should at least make an introduction, followed by some statement explaining why you've approached. Try making one or two comments relevant to the setting to display that you're not obsessing and can be casual. Then, if you want to get to the point you can say "well, listen, I don't want to take up too much of your time right here and now because I can see you were in the middle of something, but I was wondering if you'd be willing to join me for [coffee/drinks/dinner/whatever] afterward?"

You are gonna have to talk to her sooner or later, so talking is unavoidable. Even if you said: hey wanna go out, if she accepts you still have to talk when you go out
