Yes, mine was rejected three times. I had to add VIN numbers for mine and my ex's vehicle on one, small details on the other two. Wasn't a big deal for mine.

May be something technical like missing signature or missing local court form. Take a look at Return Sheet - some counties will allow you to download it from court website for a small fee.

Honestly the settlement submitted probably had an error. Everything has to be on point or the courts will reject it back to your lawyer. I am in California as well and this happened to me.
