Yes, it happens. My observation is it especially happens when you're like "oh boy, I can't believe I'm going to date this girl". I have two suggestions:

Being casual when asking, even if you're showing intent. For example, there is a ton of difference in communication between "if it is ok for you, I can take you on a date, when you're available" and "hey I kinda want to take you on a date, I think it'll be fun".
If it happens again, you can either be cool about it and understand that even if flaking dates suck, it's not like she's trying to intentionally harm you. Or, if you think that this particular girl didn't value your time and broke your trust, you can just move on and don't consider her for dating again.

You will get flaked on by tons of women. That's why people like to say "it's a numbers game". 9 out of 10 interactions with a woman won't lead to anything.

Stop asking women out. Just tell them to shoot you text and if they say "I don't have your number" then give it to them but let them initiate.

"Yes but no" is usually an indication that you might be moving too fast. I see it from their point of view now that I have some relatives that are growing up. And they are still getting familiar with themselves and how the world around them works. Boys start to like them and I think they play around with that, discovering themselves and what they are comfortable with. Socially, it means being open to speaking to most guys that approach them, but they might not actually feel comfortable doing more than that. They are still learning their boundaries. They probably have to learn how to reject them and sometimes that can be hard or awkward.

Something like this happened to me too but this one took the initiative and my phone number (so she was the more interested one) and we agreed to go hiking, then gave me an excuse and wanted to replace it; we kept texting but I felt that she was not flowing that well and I left her on seen.

Girls are this way, we like it or not, I really hate that but it is what it is.

Has happened to a lot of guys. Just keep it moving. She isn't worth it. If she doesn't want ti be with you, then leave it at that. Their is always someone willing to go on a date.
