It means that either your game is extremely good or bad. I've had women say this when I reminded them of shitty exes, cousins, brothers, fathers, and on the flip side, say this when I've reminded them of a player they used to fuck, a movie star, or someone they were generally close to.

In order to actually ascertain which end you sit on, you have to actually flirt and escalate sexually. You cannot get around this. It's the only reliable indicator of her interest. If she's receptive, you're likely on the player/comfort end, if not, it's likely you're a brother/cousin.

Also, when women speak of familiarity, they most often speak of the kind of energy and vibe you have. It's almost never based on looks.
